Southern Wisconsin Center (SWC) is recruiting for multiple Licensed Practical Nurses (LPN), located in Union Grove. Current openings are 60% FTE (48 hours every two weeks) and 90% FTE (72 hours every two weeks.) 

Shifts Available: 

  • 60% or 90% Shift: 0630 – 1900 and includes every other weekend. 
  • 90% Shift: 1830 – 0700 and includes every other weekend.    

This recruitment may be used to fill future vacancies on all shifts and FTEs.

SWC is a state operated Intermediate Care Facility for Individuals with Intellectual Disabilities. SWC provides a wide variety of programs and services designed to assist and prepare individuals to live successfully in the setting that best meets their needs. Position Summary

 

The LPN is responsible for providing safe and competent nursing care to persons who have acute and/or chronic conditions with intellectual and developmental disabilities; and works collaboratively with nursing and medical staff, interdisciplinary team members, other health care providers, families, and guardians. LPNs are accountable for:

  • Providing prescribed medications and treatments
  • Observing for changes in health conditions
  • Following the plan of care and supporting active treatment programs including basic health care and activities of daily living. 

Salary Information

 

Starting salary is between $28.56 and $36.72 /hour, based on years of licensure. Additional increases are based on years of licensure in the chart below.

  • $.80 /hour night differential and an $.80 /hour weekend differential applies to worked shifts. 
  • $2,000 sign-on bonus applies to those with no prior state service in the last five years; who stay with the facility in the position until completing probation. Bonus will be split, $1,000 upon hire and $1,000 after completing probation. 
  • 12-month probationary period is required. Pay schedule and range is 06/05. 
  • Pay for current or former state employees will be set based the rules that apply to compensation for the applicable transaction.

Job Details

 

Due to the nature of the position, all applicants who may be appointed will be required to allow DHS to conduct a background check to determine whether the circumstances of any conviction may be related to the job being filled.

All appointees will be required to receive a TB test, which will be administered by SWC or medical personnel approved by the facility prior to their start date.

This position has physical requirements which may include but are not limited to lifting up to 55 lbs, standing and walking on hard surfaces, kneeling, stooping, bending, reaching, squatting, pushing, pulling, climbing stairs and twisting. A physical assessment may be required prior to hire.  

DHS does not sponsor work visas for this position, thus, in compliance with federal law, selected candidates will be required to verify eligibility to work in the United States by completing the required I-9 form upon hire.

Qualifications

 

Minimally qualified candidates will have:

  • Licensure or eligibility to obtain a Licensure as a Practical Nurse eligible in the State of Wisconsin in accordance to the Department of Safety & Professional Services.  Licensure must be obtained by time of appointment. 
  • Experience providing prescribed medications and treatments
  • Experience making pertinent visual observations indicative of illness or dysfunction

Well qualified candidates will also have:

  • Experience transcribing medical orders
  • Experience working within an interdisciplinary team 
  • Experience working with individuals with Intellectual and Developmental Disabilities.
